A systematic review of the role of community pharmacies in improving the transition from secondary to primary care

British Journal of Clinical Pharmacology

This review of 14 studies concludes that interventions by a community pharmacist can improve drug related problems after discharge from secondary care. However, further studies are required which include process evaluations to describe the context of the intervention.
